Significant changes shall include but not be limited to...This requirement is not met as evidenced by: Based on record review, R1 did not have a reappraisal after the fall on 12/20/2025 and appraisal needs service plan was not updated after R1's incident on 12/20/2025 which poses/posed a potential health, safety or personal rights risk to persons in care.
