Annual routine visit with medical professional

Based on record review, the licensee did not comply with the section cited above in 2 out of 3 residents (R1 and R2) have not received an annual routine visit with a licensed medical professional in the last twelve months which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on LPA's/Licensee observation, interview and record review, the licensee did not comply with the section cited above by not conducting a disaster drill within the last quarter which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on LPA's/Licensee observation, interview and record review, the licensee did not comply with the section cited above in two out of three staff do not have a current CPR/1st aid certificate which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on LPAs/Licensee observation, interview and record review, the licensee did not comply with the section cited above by not paying their licensing fees in the amount of $495 which poses/posed a potential health, safety or personal rights risk to persons in care.

Maintain facility in clean, safe, sanitary condition

Based on LPA's/Licensee observation, interview and record review, the licensee did not comply with the section cited above in one out of three sinks in the main bathroom located in the hallway had a sign of "out of order". Also, garbage cans located in the bathrooms did not have tight-fitting covers in the containers, some deck boards needed to be replaced which poses/posed a potential health, safety or personal rights risk to persons in care.

Document required staff training and orientation

Based on record review, the licensee did not comply with the section cited above in four staff records were found with the most recent training of November 2022, which poses a potential health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above in not paying the annual fee which as of 12/28/2023 equals a total of $1237.00, which poses a potential health, safety or personal rights risk to persons in care.

87464(f) - Basic services shall at a minimum include care and supervision as described inHealth and Safety Code section 1569.2(c). These requirements were not met as evidenced by: Based on a review of records, it is not indicated that R1 requires the use of continence care items. Based on observations, interviews with residents, staff and outside parties, it was found that the facility uses disposable briefs and pads for R1 without documentation of needed use or care plan. In addition, outside parties had observed R1 left in soiled disposable briefs, pads and bedding on multiple occasions. This serves as an immediate health, safety or personal rights risk to residents in care.

Regular observation and documentation of resident changes

87466: Observation of the Resident. Licensee shall ensure that residents are regularly observed for changes in physical, mental, emotional & social functioning & appropriate assistance is provided when such observation reveals unmet needs. When changes such as...deterioration of mental ability or a physical health condition are observed, Licensee shall ensure that such changes are documented & brought to the attention of the resident's physician & the resident's responsible person, if any. Requirement is not met as evidenced by: Based on Department investigation, resident (R1) sustained mulitple pressure injuries and severe injury resulting in amputation which poses an immediate health and safety risk to residents.

Limit postural support devices to mobility needs

87608(a)(1) Postural Supports. Postural supports may be used to achieve proper body position and balance, improve mobility and independent functioning or to position rather than restrict movement. This requirement is not met as evidenced by** Based on interviews with Licensee and observations; LPA found that facility utilizes waist strap/harness for resident (R1) without physician's orders. This poses an immediate Health, Safety or Personal Rights risk to Clients in care.

Based on observation, the licensee did not comply with the section cited above in 1 out of 6 auditory alarms which poses/posed a potential health, safety or personal rights risk to persons in care.

Fire approval and staff access to unlock systems

Based on observation, the licensee did not comply with the section cited above from observations of 2 bottles of bleach, and garage contaning additional potentially dangerous substances accessible to residents in care which poses an immediate health, safety or personal rights risk to persons in care.
