Facility personnel sufficiency and competence

87411 Personnel Requirements - General (a) Facility personnel shall at all times be sufficient in numbers, and competent to provide the services necessary to meet resident needs… This requirement was not met by licensee as evidenced by: Based on investigation, interviews, photographic evidence, and observation of R1, due to staff severe neglect, resident sustained a blister on their leg, which posed an immediate health, safety, and personal rights risk to resident in care.

Regular observation and documentation of resident changes

87466 Observation of the Resident The licensee shall ensure that residents are regularly observed for changes in physical, mental, emotional and social functioning and that appropriate assistance is provided when such observation reveals unmet needs… This requirement was not met by licensee as evidenced by: Based on CCL Investigation, R1 was not treated for their burn until four days after it was first discovered by staff and only after the heat blister ruptured.

Based on observation, the Administrator did not comply with the section cited above in 1 out of 1 fire extinguisher was not serviced since February 2021 which poses an immediate health, safety or personal rights risk to persons in care.

This requirement had not been met as evidence by: Based on records review of facility self-incident report dated 9/9/22 and interviews conducted with Administrator, the facility staff (S1) on 9/8/22 incorrectly assisted resident (R1) and given an extra dosage of medication Lamotrigine 25mg which poses an immediate risk to the health and safety of the clients in care.

This requirement had not been met as evidence by: Based on records review of facility self-incident report dated 6/4/22 and interviews conducted with Administrator, the facility staff (S1) on 6/2/22 incorrectly assisted resident (R1) and given an extra dosage of prescribed medication Lamotrigine 25mg which poses an immediate risk to the health and safety of the clients in care.
