What the inspector wrote

On 10/1/2025, Licensing Program Analyst (LPA) Triel Ellen Lindstrom arrived unannounced at the facility to conduct a case management visit related to recent elopements at the facility that were self-reported to the Department on two separate Unusual Incident/Injury Reports (LIC624). LPA Lindstrom met with the Memory Care Director, explained the purpose of the visit, and conducted an interview. Since the elopement, management has replaced the door on the back fence of the Memory Care Unit's garden. The new door has an egress with a lock that stays locked when pushed and sounds an alarm. An exit interview was held with the Memory Care Director and a copy of this report was provided.
