This report shall include the resident's name, age, sex and date of admission; date and nature of event; attending physician's name, findings, and treatment, if any; and disposition of the case.This requirement was not met as evidenced by: Based on file review and interview, The licensee did not ensure that facility reported R1's falls that occured on 06/18/2024 and 07/26/2024 were reported to the department. LPA reviewed facility records and found that there were no reported incidents regarding falls or R1 within the months of March 2024-August 2024. This poses an immediate health,safety and personal rights risks to persons in care.
