What the inspector wrote

At 11:54 A.M. Licensing Program Analysts (LPAs) Valeria Conway and Kelly Dulek arrived at the facility unannounced to conduct a case management visit to follow up on the relocation of Resident #1 (R1). The LPAs met with Licensees Teresita and Larry Wayne and discussed the reason for the visit. At the time of the visit, the facility had only one (1) resident in care. The administrator stated that they are currently in the process of finding an alternative placement for the resident. The LPAs, along with facility Licensee Larry Wayne, toured the physical plant areas inside and outside to ensure there are no health and safety hazards. Additionally, LPAs reviewed R1's home health records. Following the last meeting with Community Care Licensing Staff, the Licensee stated that Adult Protective Services (APS) visited R1 on two (2) separate occasions; however, R1 refused their assistance. The licensee also reported that R1 declined to visit a physician to obtain an assessment that would support the relocation process. Furthermore, Licensee, reported attempting to contact R1's out-of-state family member; however, the phone number on record was found to be disconnected. Regarding the current utility services being shut off, the licensee stated that billing is not a concern at this moment. During today's visit, the licensee inquired about the unlawful detainer process in relation to the upcoming 60-day facility closure effective August 13, 2025. LPAs explained the process to the licensee. LPAs obtained relevant information and provided the licensee, via email, with a comprehensive list of facilities that may potentially admit R1. No citations issued. Exit interview conducted and report provided to Licensee Larry Wayne.
