What does Type A vs Type B mean?
Type A. Serious citation. Imminent or substantial risk to children. The regulator requires corrective action immediately and may impose a civil penalty.
Type B. Lower-severity citation. Corrective action required, no imminent risk. The regulator monitors compliance on the next visit.

(a)Each licensee shall furnish to the licensing agency reports…within seven days of the occurrence of any of the events specified in (A) through (D). (D)Any incident which threatens the welfare, safety or health of any resident. This requirement was not met as evidenced by: Based on record review, the licensee did not comply with the section cited above as, R1 had an unwitnessed fall causing a forehead bruise and right knee swelling which prompt hospice to have R1 placed on bed rest, which posed a potential health and safety risk to resident in care.

(f)Basic services shall at a minimum include: (1) Care and supervision as defined in Section 87101(c)(3) and Health and Safety Code section 1569.2(c).This requirement is not met as evidenced by: Based on the information obtained and reviewed, the Licensee did not comply with the section cited above as, R1 was left unsupervised resulting in R1 falling off their wheelchair and sustaining injuries, which posed an immediate health and safety risk to resident in care.

(a) Residents in all residential care facilities for the elderly shall have all of the following personal rights: (2) To be accorded safe, healthful and comfortable accommodations, furnishings and equipment.This requirement is not met as evidenced by: Based on the information obtained and reviewed, the Licensee did not comply with the section cited above as, staff failed to report faulty wheelchair brakes to hospice causing R1 to have an unwitnessed fall in the dining room, which posed a potential health and safety risk to resident in care.
