What does Type A vs Type B mean?
Type A. Serious citation. Imminent or substantial risk to children. The regulator requires corrective action immediately and may impose a civil penalty.
Type B. Lower-severity citation. Corrective action required, no imminent risk. The regulator monitors compliance on the next visit.

Facility personnel shall at all times be sufficient in numbers, and competent to provide the services necessary to meet resident needs… All persons who supervise employees or who supervise or care for residents shall be at least eighteen (18) years of age. This requirement was not met as evidenced by: Based on record review and interviews, the Licensee did not comply with the section cited above as S1 was assisting residents with ADLs but was not eighteen (18) years of age, which posed an immediate health and safety risk to persons in care.

A plan for incidental medical and dental care shall be developed by each facility. The plan shall encourage... and provide for assistance in obtaining such care, by compliance with the following: The licensee shall assist residents with self-administered medications as needed. This requirement was not met as evidenced by: Based on record review, the Licensee did not comply with the section cited above as R1’s MAR indicates medication is being administered but medication was not found and R2 has missing PRN as staff has not reordered yet, which posed an immediate health and safety risk to residents in care.
