What the inspector wrote

Licensing Program Analysts (LPAs) Kelly Dulek and Valeria Conway conducted an unannounced visit at the facility in conjunction with an investigation of complaint control # 29-AS-20250519143156. During the investigation, LPAs observed deficiencies unrelated to the complaint allegations. During facility tour, which began at 10:47AM, LPAs observed both delayed egress gates from the memory care were closed and unable to be opened. The gate on the north side of the memory care was observed to be wired shut at the bottom and have a bungee cord attaching the exit gate to the fence, preventing the gate from opening. The other exit gate was observed to be locked with a key. LPAs also observed feces outside in the memory care courtyard. Pursuant to Title 22 of the CA Code of Regulations, the following deficiencies were cited (refer to LIC 809-D). Exit interview conducted, today's reports and appeal rights were reviewed and issued.

    (a) The facility shall be clean, safe, sanitary and in good repair at all times. Maintenance shall include provision of maintenance services and procedures for the safety and well-being of residents, employees and visitors.This requirement is not met as evidenced by: Based on LPA observations, the licensee did not comply with the section cited above as feces was observed in the courtyard of the Meadows accessible to residents in care, which poses an immediate health and personal rights risk to persons in care.
