What the inspector wrote

Licensing Program Analyst (LPA) Valeria Conway conducted an unannounced case management visit to issue an amended report. LPA met with Executive Director, Monica Reyes, and explained the purpose of the visit. On 05/09/2024, a 1-year required annual inspection was conducted. During the annual visit, citations were issued for Fire Clearance and Criminal Record Clearance. Based on information provided in an appeal, the 05/09/2024 citations are being revised. Please refer to LIC 809-D issued today for the amended citations. Exit interview, report given, deficiencies cited on 809-D.
