What the inspector wrote

Licensing Program Analysts (LPAs) Valeria Conway and Kelly Dulek conducted a Case Management - Deficiencies visit in conjunction with a complaint visit conducted on today's date. LPAs met with facility designee, Agnes Gazaryan. The purpose of this report is to issue citations for a deficiency observed during the complaint investigation. During today's visit, the Administrator of record was unavailable. LPAs observed that the LIC 500 Personnel Report indicates the Administrator is scheduled Sundays and Mondays from 09:00AM to 09:00PM. However, the Administrator was not present on today's visit, nor the visit conducted on 01/26/2026, which was also a Monday. Interviews with staff revealed the Administrator has not been present in the facility in about a month or two (2) months. Pursuant to Title 22, California Code of Regulations and/or CA Health and Safety Code, the following deficiency was cited (refer to LIC 809-D.) Facility Designee was informed that failure to correct the deficiency may result in civil penalties. Exit interview conducted, appeal rights discussed, and a copy of this report and appeal rights were provided.
