Based on record review, the licensee did not comply with the section cited above as Resident 1 (R1) resident did not have a current appraisal/ needs and service plan on file which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on record review and interview, the licensee did not comply with the section cited above as R3's medications were being given eventhough they had been discontinued; one of their medications had not been filled, and start dates for medications were documented eventhough medications were not being given, which poses an immediate health, safety or personal rights risk to persons in care.

Based on observation, the licensee failed to ensure the hot water temperature was within the required range (tested between 150.2-155.5 degrees F) which poses an immediate health and safety risk to persons in care.

Based on medication review, the licensee did not comply with the section cited above for 1 (one) out of 3 (three) residents in which a prescribed melatonin medication indicated for bedtime use was administered in the morning before the visit which poses an immediate health, safety and personal rights risk to persons in care.

Based on observation, the licensee did not comply with the section cited above in that 1 (one) out of 2 (two) side gates do not self-latch which poses a potential health and safety risk to persons in care.

Based on record review, the licensee did not comply with the section cited above in that 3 (three) out of 6 (six) residents did not have an updated/current appraisal needs and services plan which poses a potential health, safety and personal rights risk to persons in care.

87202 Fire Clearance (a) All facilities shall maintain a fire clearance approved by the city, county, or city and county fire department, or district providing fire protection services, or the State Fire Marshal. Prior to accepting or retaining any of the following types of persons, the applicant or licensee shall notify the licensing agency and obtain an appropriate fire clearance approved by the city, county, or city and county fire department, or district providing fire protection services, or the State Fire Marshal.(2)Bedridden persons.This requirement is not met as evidenced by:

Based on observation, the licensee did not comply with the section cited above as in the water temperature in resident bathrooms measured between 132.3 and 148.4 degrees F which poses an immediate health, and safety risk to persons in care.

Based on observation, the licensee did not comply with the section cited above, as staff were observed without the appropriate face covering, which poses a potential personal rights risk to residents in care.

87705 Care of Persons with Dementia. (c)(5) Each resident with dementia shall have an annual medical assessment as specified in Section 87458, Medical Assessment, and a reappraisal done at least annually, both of which shall include a reassessment of the resident’s dementia care needs.
