What providers tell us about this citation

Based on community experience, not official guidance.

230 California RCFEs have a Type A citation here. Assign one staff member to own a medical and dental appointment log, and keep written proof you arranged each visit. LPAs ask for that file early, and an empty one turns a routine review into a Type A finding.

FAQ

Frequently asked questions

Answers based on public CCLD data and regulation text. May not reflect recent changes.

What is a medical and dental care violation in an RCFE?
It means the facility failed to arrange, or help arrange, medical or dental care that a resident's condition needed. Title 22, Section 87465(a)(1) makes this the licensee's duty, not the family's alone. When a resident's worsening health goes unaddressed, the result can be hospitalization or untreated pain. CCLD treats this as a direct risk, which is why it lands as a Type A citation.
How common is this violation in California assisted living?
It is more frequent than many operators expect. According to California CCLD inspection records, 230 California RCFEs have been cited under Section 87465(a)(1), the large majority as Type A violations carrying direct risk to residents. Los Angeles leads with 44 citations, followed by Sacramento with 37 and Orange County with 30. Even well-run communities appear on this list.
What happens if an RCFE is cited for failing to arrange care?
Because this is a Type A citation, it signals immediate risk and draws closer scrutiny than a Type B finding. The LPA documents the deficiency, sets a correction date, and schedules a follow-up visit to confirm the fix. Type A citations carry higher civil penalties than Type B citations. Repeated or uncorrected findings can escalate to formal enforcement against the license.
How do I fix or prevent this violation?
Build a tracking system for every resident's medical and dental appointments. Assign one staff member to log physician orders, schedule visits, and arrange transportation. Review each resident's record during the required reappraisal so changing conditions trigger new care. Keep written proof that you arranged or offered care, including dates and provider names. LPAs ask for this paper trail, so a clear file answers the question before it is asked.
Does this violation affect my RCFE license?
Yes, it can. CCLD records every citation in your facility file, and a Type A finding for unmet medical care weighs heavily during license review and complaint investigations. A single corrected citation rarely threatens your license on its own. A pattern of unaddressed resident health needs is a different matter and can support suspension or revocation. Correct quickly and document the fix.
