What the inspector wrote

Licensing Program Analyst (LPA) Kassandra Medrano made an unannounced inspection. LPA Medrano was granted entry by Teacher, Juanita Simmons. At 10 am, present in the facility were 19 children (18 preschool age and 1 school age child) with one teacher. The purpose of the inspection was explained for Case Management report due to deficiencies found. LPA Medrano toured the facility and inspected for health and safety hazards. At 10:37am, S3 arrived to the facility. It was later observed that S3 was supervising 11 children independently. A Type “A” violation was issued today. The center was advised to provide a copy of the Evaluation Report and the Type “A” Deficiency cited to the parents and guardians of children currently enrolled in care and to parents of newly enrolled children during the next 12 months. LIC 9224 must be signed and dated within the next business day the children are in care and are to be kept in the children’s files. This report and appeal rights were provided and reviewed with the Site Director. Notice of Site Visit and Type A Citation shall remain posted for 30 days. Exit interview conducted, report, and appeal rights were reviewed with the facility representative, Juanita Simmons.

    101216.2 Teacher Aide Qualifications and Duties(e) An aide shall work only under the direct supervision of a teacher.This requirement was not met as evidenced by: Based on observations S3 was left alone supervising 11 children. This poses a potential health and safety risk to children in care.

    101216.3 Teacher-Child Ratio(a) There shall be a ratio of one teacher visually observing and supervising no more than 12 children in attendance...(b) The licensee may use teacher aides in a teacher-child ratio of one teacher and one aide for every 15 children in attendance. This requirement was not met as evidenced by: Based on observations and records reviews it was found that S1 was alone with 19 children. S3 later arrived at 10:37am. The facility still remained out of ratio due to the unqualified staff. This poses an immediate health and safety risk to children in care.

    INDOOR ACTIVITY SPACE FOR SCHOOL-AGE CHILDREN

    101538.3 Indoor Activity Space for School-Age Children(b) In combination programs, indoor activity space provided for school-age child care center children shall be physically separated from space provided for infant care and child care center children.This requirement was not evidenced by: Based on observations and interviews of staff it was found that facility does not separate school age and preschool children. This poses a potential health and safety risk to children in care.
