On 11/02/2022 Licensing Program Analyst (LPA) Arminder Singh and Licensing Program Manager (LPM) Loretta Dyson arrived at facility to conduct a unannounced case management visit. LPA and LPM met with Owner, Lakesha D. Arrif. Present during visit were two children, lead teacher, and aide. This visit is in regards to facility not notifying CCLD regarding the outbreak of hand, foot, and mouth outbreak and the pink eye outbreak. The Owner and staff have stated that they did notify the parents of the outbreaks at facility. The Owner has provided copies of the two exposure notices that were given to the parents. The facility did not inform CCLD regarding the two outbreaks. Facility upon occurrence to the outbreak a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours. In addition, a written report containing the information shall be submitted to the Department within seven days following the occurrence of such events. The Owner stated she did not remember that a report was to be made by telephone or fax within the next working day in addition a written report was to be submitted to CCLD within seven days following the outbreaks. During the visit it was discovered that facility does not have a Director currently at facility. The Owner advised that individual that is being hired for the Director position is currently out due to medical reasons and shall return back to facility prior to the winter break. Owner has confirmed that the facility has not had a Director since June 2022. Owner has been acting as the Director in the meantime. see page 809-D for Deficiencies.
