What Happened
The plan-of-correction follow-up found staff supervising too many preschool children and repeatedly being left alone with the group. The analyst also found that the teachers did not have documentation showing they were qualified to be left alone with children, and the required report postings and acknowledgements were still missing.
Observations
- Staff members Ashley Adams and Sandra Morando were alone with preschool children on arrival.
- Sandra Morando was observed alone with 14 to 16 children during the visit.
- Ashley Adams was observed alone with 14 to 15 children during the visit.
- File review showed neither teacher had documentation proving they were qualified to be left alone with children.
- The Non Compliance Conference report was not posted.
- Three unsigned LIC 9224 acknowledgements were found for the prior Type A citations.
