What the inspector wrote
On 9/21/2023 at 9:00 AM Licensing Program Analysts (LPAs), A. Curry and B. Crass arrived at the home and conducted an announced Prelicensing inspection. LPA met with applicant, Miledi Silverio, who submitted an application for a Large Family Child Care Home. Also present for the inspection is the applicant\342\200\231s husband Jose Quezada and applicant's infant child. The hours of operation for the family child care home will be Monday-Friday 6:00 AM-6:00 PM. A tour of the home was completed with the applicant to conduct a health and safety inspection.
The home is two stories and consist of 3 bedrooms, 2 bathrooms, living room, kitchen, mudroom, fenced front yard, converted garage space, and laundry room, which is located inside the downstairs bathroom. The children will use the downstairs bathroom, living room, kitchen, mudroom, front yard, and converted garage space. The applicant is aware that day care children shall not eat or sleep in the converted garage space. The converted garage space shall solely be used as a play area. Areas accessible to children were inspected to ensure that they are clean and orderly with ventilation and heating for safety and comfort.
The off-limits areas are the down stairs bedroom, laundry room, and the entire upstairs, which consist of 2 bedrooms and 1 bathroom.
The off-limit areas will be inaccessible by closed and/or locked doors and visual supervision

The
Isolation area will be in an area in the living room away from the other children. The applicant was advised that any area that will be changed to an on limit area will need to be inspected by Licensing prior to use. There were safe toys, play equipment, and materials observed for children. LPA did not observe any bodies of water, toxins, medications or hazardous items that would be accessible to children. The applicant was advised that all poisons and/or hazardous items must be kept in a locked cabinet/drawer or placed out of reach of children. There is a safety gate at the bottom of the stairs to prevent access to the upper area of the home. The applicant stated there are no firearms on the premises.
The home is equipped with a fully charged 3A40BC fire extinguisher, working smoke alarm, working carbon monoxide detector, telephone, and first aid supplies. The applicant completed the required preventative health and safety training, which includes 1 hour of nutrition, the required lead poisoning prevention training, and has current CPR and First Aid training which expires on December 11, 2024 respectively.
The applicant was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated. \\
The applicant provided proof of control of property. Because the applicant rents/leases the home, proof of landlord notification is required. The LPA observed the Property Owner/Landlord Notification form (LIC9151) that the applicant confirms form was provided to the property owner/landlord. The applicant obtained a signed Property Owner/Landlord Consent form (LIC 9149).

LPA reviewed with applicant the LIC 311D, Forms/Records to Keep In Your Family Child Care Homes, children\342\200\231s forms/records, facility forms/records, and information to be posted. Entrance Checklist was provided to the applicant
LPA discussed the safe sleep regulations with applicant and discussed the Child Care Licensing Safe Sleep webpage at:

, as an additional resource. LPA also informed [applicant, licensee, or facility representative] of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at:

, and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.
On this date, 09/21/2023, the California Attorney General - Megan\342\200\231s Law website was searched for information on sex offenders required to register with local law enforcement under California's Megan's Law. No registered sex offenders were found at the facility addresses. Under state law, some registered sex offenders are not subject to public disclosure; therefore, they may not have been included in this search. However, the Department conducts a monthly cross reference of each address on record for all registered sex offenders against all CCLD facility addresses pursuant to information shared by California DOJ.
Applicant was informed of the
MyChildCarePlan.org
site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.
The license will be effective today, 09/21/2023.
A notice of site visit was given to the applicant and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days.
