What the inspector wrote

Licensing Program Analyst (LPA) Jyoti Saini conducted an unannounced case management today and met with Director Shubra Gupta. Present during the inspection were the director, assistant director, and seven (7) staff members supervising 45 children. During an investigation of the concern brought into department’s attention, interviews revealed a connection to a prior incident from February 2025. It is determined that the facility had misrepresented the nature of the injury to parents. These revelations lead to the conclusion that the facility failed to uphold its responsibility to ensure the safety of individuals receiving services, therefore a Type B citation is being cited. California Code of Regulations, {Title 22, Division 12, Chapter 1, Section 101206(a)(c) is being cited on the attached LIC809-D. Appeal rights were given. The notice of site visit was provided and must remain posted for 30 days. An exit interview was conducted, and the report was reviewed with the Director, Shubra Gupta.
