What the inspector wrote

Licensing Program Analyst (LPA) Jyoti Saini and Licensing Program Manager (LPM) Wynn Norona arrived unannounced to deliver the findings from a complaint investigation for the above allegation. LPA and LPM met with interim Director Amy Quintar and Regional Director, Eman Isbeih and explained the purpose of the inspection. Present during today’s visit, interim director and three (3) staff members supervising 16 children. Based on the interviews conducted, facility management did not acknowledge that such an incident occurred and interviews with staff revealed discrepancies. Although the facility is equipped with surveillance cameras, the regional director stated that the system is currently undergoing transition to a different IT provider, and therefore no recordings are available, leading to the conclusion that while the allegation may have occurred or is valid, there is no preponderance of evidence to prove the alleged violation. Therefore, the allegation is "Unsubstantiated. Appeal rights were provided. An exit interview was conducted, and the report was reviewed with Regional Director Eman Isbeih. A notice of site visit was posted and must remain posted for 30 days.

(1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation.This requirement is not met as evidenced by: Based on interviews, and record reviews, the Licensee did not comply with the section cited above, as of April 15, 2025, the lack of visual supervision resulted in a delayed response to a medical emergency, which poses an immediate risk to the health, safety, and personal rights of the children in care.

    101230 Activities (c) A teacher-child ratio of one teacher supervising 24 napping children is permitted provided that the remaining teachers necessary to meet the overall ratio specified in Section 101216.3(a) are immediately available at the center.This requirement is not met as evidenced by: Based on interviews and record reviews, the Licensee did not comply with the section cited above. Staff member S1 was observed supervising a group of 14 children during the scheduled nap period. Out of the 14 children, two were awake and active, which placed the teacher out of ratio. This staffing arrangement poses a potential risk to the health, safety, and personal rights of the children in care.

(c) Conduct which is inimical to the health, morals, welfare, or safety of either an individual in or receiving services from the facility or the people of this state.This requirement is not met as evidenced by: Based on observations, interviews, and record reviews, the Licensee did not comply with the section cited above, as discrepancies were identified in the children's records maintained at the facility, which poses a potential risk to the health, safety, and personal rights of the children in care.
