What the inspector wrote

On 10/14/2025 Licensing Program Analyst (LPA) Jyoti Saini and Licensing Program Manager (LPM) Wynn Norona conducted an unannounced case management inspection at the facility. LPA and LPM met with interim Director Amy Quintanar and Regional Director, Eman Isbeih. During the investigation into the complaint allegations received on April 16, 2025, interviews and record reviews revealed a discrepancy in the documentation related to the Toddler Program Option maintained in Child C1’s file. The version of the form obtained during the investigation by IB Investigator did not match the version provided to the police department. This discrepancy raises concerns regarding whether the form was signed during the enrollment process or after the incident occurred. These inconsistencies in record keeping practices constitute conduct inimical to the health, safety, and welfare of children in care, as defined under Title 22 regulations. As a result, California Code of Regulations, Title 22, Division 12, Chapter 1, Section 101206(a)(c), is cited on the attached LIC 9099D along with a Civil Penalty assessment for a repeat violation within 12 months. The facility was cited under the same citation on 06/04/2025. Appeal rights were provided. An exit interview was conducted, and the report was reviewed with Regional director, Eman Isbeih A notice of site visit was posted and must remain posted for 30 days.

(1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation.This requirement is not met as evidenced by: Based on interviews, and record reviews, the Licensee did not comply with the section cited above, as of April 15, 2025, the lack of visual supervision resulted in a delayed response to a medical emergency, which poses an immediate risk to the health, safety, and personal rights of the children in care.

    ACTIVITIES/NAPPING

    101230 Activities (c) A teacher-child ratio of one teacher supervising 24 napping children is permitted provided that the remaining teachers necessary to meet the overall ratio specified in Section 101216.3(a) are immediately available at the center.This requirement is not met as evidenced by: Based on interviews and record reviews, the Licensee did not comply with the section cited above. Staff member S1 was observed supervising a group of 14 children during the scheduled nap period. Out of the 14 children, two were awake and active, which placed the teacher out of ratio. This staffing arrangement poses a potential risk to the health, safety, and personal rights of the children in care.

    101206 Revocation or Suspension of License (a)The Department has the authority to suspend or revoke a license on any of the grounds specified in Health and Safety Code Section 1596.885. (c) Conduct which is inimical to the health, morals, welfare, or safety of either an individual in or receiving services from the facility or the people of this state.This requirement is not met as evidenced by: Based on observations, interviews, and record reviews, the Licensee did not comply with the section cited above, as discrepancies were identified in the children's records maintained at the facility, which poses a potential risk to the health, safety, and personal rights of the children in care.
