Large facility with multiple record deficiencies: one child with incomplete physician's report, three staff missing immunization records, and four staff with expired or missing mandated reporter training.

1-year inspection identified Type A violation: Lysol toilet bowl cleaner accessible to children in yellow yaks classroom. Additionally, one child's inhaler was incorrectly labeled in the same room. Facility has completed lead testing and is remediating lead exceedances.

Case management visit for multiple elevated lead levels in drinking water. Three faucets exceeded safe lead thresholds (8.5, 10, and 15 ppb). All were made inaccessible and children receive water from safe filtered and lead-free sources.

Complaint investigation found substantiated violations: center failed to obtain signed acknowledgment of receipt from parents for licensing reports, and child unsupervised in classroom with scissors. Unsubstantiated allegations regarding bathroom incident and hallway hazard.

Incident investigation found child left classroom and exited outside facility doors unsupervised during pick-up time (less than 1 minute). Parent noticed child and brought them back.

2-year-old child left unattended in bathroom for approximately 12 minutes during classroom transition. Child discovered outside by incoming parent. Type A violation issued for inadequate supervision.

Follow-up inspection for COVID-19 mask compliance found facility in continued violation: 49 children observed indoors without required facial coverings, demonstrating ongoing failure to enforce State mandate following previous citation on 9/21/21.

Plan of Correction inspection to verify compliance with Type A citation from 9/21/21. Finding: alleged violation not corrected. LPAs observed 51 children in 4 classrooms not wearing face coverings. Licensee Tamara Wanink stated children are given choice regarding mask wearing and facility is not enforcing State mask mandate.
