What the inspector wrote

On 3/5/26 @ 8:17am an unannounced case management inspection was conducted by Licensing Program Analyst (LPA), Erica Laird. LPA met with facility director, Julia Willette in response to an Unusual Incident Report received by the Department on 3/4/26. Per the incident report, a child (C1) fell from a play structure resulting in a concussion. Facility director Julia Willette was interviewed on 3/5/26 @ 9:07am and stated (C1) was climbing on the volcano structure when they lost their footing and fell. Julia stated she was informed later that day that C1 was suspected of having a mild concussion. During today’s inspection, the facility was toured and two staff (S1-S2) were interviewed. S1 state they were present when C1 fell. S1 stated C1's foot was caught causing them to slowly slide down the structure. S1 stated they contacted C1's parent and C1 was sent home. On 2/25/26 LPA Laird received the manufacturing guidelines for the volcano climbing structure. Manufacturing guidelines indicate the structure is for family and domestic use only and is to be used by children ages 4-8 years. Based on observations and manufacturing guidelines, the following deficiency(ies) is being cited on the LIC809-D. Age appropriate play equipment 101239(m). LPA Erica Laird informed director Julia Willette that this report dated 3/5/26 documents 1 Type A citation which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care. LPA Erica Laird informed director to provide a copy of this licensing report dated 3/5/26 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Exit interview conducted and report was reviewed with the facility director, Julia Willette. Appeal Rights were provided. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
