What the inspector wrote

An unannounced case management inspection was conducted today 11/25/25 at 11:46am by Licensing Program Analyst (LPA), Tammy Dutra. LPA met with facility representative Lizbeth Navarro in response to an Unusual Incident Report received by the Department on 9/25/25. A child in care, (C1) fell on the play structure and hit above their eye causing a cut which required stitches. The facility representative was interviewed on 10/14/25 at 2:14pm and stated that they were not present on the playground during the incident. They were informed about the incident and staff indicated they thought C1 needed medical attention. Facility contacted the parent for C1 to be picked up. Facility representative stated that C1 returned to school the next day and resumed normal activities. LPA attempted to contact parent of C1 and was not able to reach them. C1 returned to school the following day and no further issues arose from the injury. LPA spoke with the staff member, S1 present during the incident who said C1 was spinning around a pole located on the play equipment. S1 stated they were actively supervising C1 and attempted to stop them from spinning before the accident occurred. S1 immediately removed C1 from the playground after discovering the injury and applied first aid. Once C1 received first aid, S1 attempted to contact C1's parent to seek medical attention. S1 said C1 returned to school the following day with stitches. Once stitches were removed no further medical attention was needed. LPA toured the playground and inspected the outdoor play equipment. Outdoor play equipment was age appropriate and free of hazards. During a previous case management conducted on 10/14/25 LPA interviewed staff, observed the play equipment and reviewed the steps staff took to prevent the incident. Staff took appropriate actions to ensure C1 was given first aid and parent was notified in a timely manner. Facility was operating within the licensed capacity and ratio requirements. Based on interviews, observation and record review LPA determined the incident was an accident and could not have been prevented. Exit interview conducted and report was reviewed with the facility representative, Lizbeth Navarro. Appeal Rights were provided. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
