What the inspector wrote

An unannounced case management inspection was conducted today at 3:30pm by Licensing Program Analyst (LPA), Tammy Dutra. LPA met with facility representative Jaquelin Hernandez in response to an Unusual Incident Report received by the Department on 1/13/26. A child in care, C1 was injured on the play structure and required medical treatment. The facility representative was interviewed on 1/21/26 and stated that they were not present during the incident but they believed that C1 fell on the play structure and hit their head. Facility representative believed C1 was given medical treatment and returned to school the following day. Facility representative stated that there were 8 children present with two teachers during the incident. Two staff (S1 & S2)and one child (C1) were interviewed on 1/21/26. C1 stated they were sitting on the handrail and fell backwards hitting their head on the stairs. C1 was provided first aid and when S1 realized there was blood on the back of C1's head they immediately notified C1's parent. C1 was treated by a physician and returned to school the following day. LPA interviewed C1's parent on 1/15/26 who shared that they were aware of the accident and they were able to get C1 medical care. C1's parent had no concerns regarding supervison. C1's parent shared that their child returned the following day and is awaiting a follow up from the physician. During today\342\200\231s inspection, the facility was toured and the playground equipment was observed. LPA observed the location of the incident and took photos of where the accident took place. LPA reviewed the play equipment and established it is age appropriate for children in care. LPA advised staff use the playground equipment as manufacturer's recommended and recommended the handrails not be used for sitting to prevent future accidents. Bas ed on interviews and observation LPA determined the incident was an accident and staff are taking measures to prevent future incidents. Licensee is adding extra steel coat to the existing structure to cover patches. Exit interview conducted and report was reviewed with the facility representative Jaquelin Hernandez . Appeal Rights were provided. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
