What the inspector wrote

An unannounced case management inspection was conducted today 1/29/26 at 3:02 pm by Licensing Program Analyst (LPA), Tammy Dutra. LPA met with licensee Sarah Livingston in response to an Unusual Incident Report received by the Department on 1/16/26. On 1/16/26 around 2:30pm a child in care, C1 was injured on the playground and sought medical treatment. The licensee was interviewed on 1/29/26. Licensee stated that a child in care C1 was running from the grass onto the pavement and lost their footing falling on their face. C1 was given first aid and C1's parent was called. C1's parent took them to the doctor and it was decided no treatment was needed. LPA reviewed video footage of the fall. LPA interviewed one staff member, S1 on 1/21/26 who witnessed C1 falling. S1 stated that they witnessed C1 running and lose their footing while taking a sharp turn from grass to the concrete. S1 noticed there was a large bump on C1's head and S1 informed the licensee and applied first aid. Licensee contacted C1's parent who took the child to receive medical care. S1 stated that C1 returned after the weekend with no follow up care required. S1 stated that there were 9 children present with two teachers during the incident. LPA attempted to contact C1's parent on 1/29/26 and has not received a return call. LPA attempted to interview C1 on 1/21/26 who stated they were running and fell in the location S1 pointed out. LPA was not given a reason behind the fall from C1 during the interview. During today\342\200\231s inspection, the facility was toured and the playground equipment was observed. LPA observed the location of the incident and saw no hazards that may have caused the injury. Based on interviews, video review and observation LPA determined the incident was an accident and staff are taking measures to prevent future injuries on the playground. Exit interview conducted and report was reviewed with the licensee Sarah Livingston. Appeal Rights were provided. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
