What the inspector wrote

An unannounced case management inspection was conducted today at 11:40am by Licensing Program Analyst (LPA), Tammy Dutra. LPA met with facility representative Corina Vasquez in response to an Unusual Incident Report received by the Department on 3/11/26. A witness (W1) allegedly observed a staff member (S1) shaking a crib aggressively while hearing a child (C1) crying in the crib. The facility representative was interviewed on 3/13/26 at 11:40am and stated that she was informed of the situation but she was not present during the incident. Center director was away from the facility during the visit. Two staff members (S1 & S2) were interviewed on 3/13/26 . S1 stated they were moving a crib to continue rocking a baby and patting another baby in a crib. S1 stated the crib was stuck between the chair and the swing which caused them to shake it in order to move the chair. S1 believed that W1 saw them moving the empty crib and believed there was a child in the crib. S2 stated they were in and out of the nap area during the incident but they did not see their co-worker acting aggressively and believed there was a misinterpretation of the situation. During today’s inspection, the facility was toured and LPA received a copy of the children's roster, staff clock in/out record, child attendance sheet and a sleep check for C1. LPA requested a copy of the Center's discipline policy listed in the parent handbook or employee handbook. L PA observed the area the alleged incident occurred and photographed the nap area. Due to insufficient information at this time, further investigation is needed. Exit interview conducted and report was reviewed with the facility representative Corina Vasquez. Appeal Rights were provided. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
