What the inspector wrote

On 3/5/26 @ 4:00pm LPA Erica Laird conducted an unannounced case management inspection. LPA Laird met with licensee, Ashley Jacobian. On 12/3/25 LPA Laird received five incident reports from Ashley Jacobian involving inappropriate statements and interactions between a daycare child (C1) and other children in care on 3/21/24, 7/7/24, 11/11/24, and 11/20/25. On 3/5/26 LPA Laird conducted an interview with licensee Ashley Jacobian. Ashley stated she didn't report the incidents to licensing because she didn't think the incidents were reportable. Ashley stated none of the incidents involved injuries or a serious incident so she didn't think she needed to report it to licensing. LPA Laird discussed reporting requirements with licensee, specifically Health and Safety Code 1597.467(b)(1)(C). Based on documents received on 12/3/25 and interview conducted with licensee, deficiency(ies) is being cited on the LIC809-D. Exit interview conducted and report was reviewed with the licensee, Ashley Jacobian. Appeal Rights were provided. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
