Follow-up on corrections·July 22, 2025

On 7/22/2025 at approximately 02:00PM, LPAs Maiwandi and Sykes conducted a case management inspection at Railroad Junction School. LPAs met with director Lalita Gupta, and explained the purpose of the visit. Upon arrival LPAs observed 48 children in care, (28 preschool and 20 school-age) with 5 staff in 4 classrooms. Director stated there are 55 children enrolled. All staff have current criminal record clearance. While investigating and interviewing staff and children for a inspection visit for the preschool component, LPAs obtained evidence where children were harmed in school-age classrooms while staff did not intervene. Upon further investigation, it was brought to LPAs attention from multiple interviews, C2 and C6 informed LPAs that they did not feel safe at the facility. LPAs were also informed by C7 that staff members send children to the twos classroom when misbehaving and there are no staff present. Due to the staff not meeting the children's needs in stopping altercations and leaving children in the classroom unattended, there is a lack of supervision which is an immediate risk to the health and safety of children in care. Type A deficiency is being cited for lack of supervision. LPAs J. Maiwandi and K. Sykes informed director Lalita Gupta that this report dated on 7/22/2025 documents one Type A citation which shall be posted for 30 consecutive days as there is/are immediate risk to health and safety or personal rights of children in care. Also LPAs Maiwandi and Sykes informed the director to provide a copy of this licensing report dated 7/22/2025 that documents any type A citations to parents/guardians of all children currently enrolled by the next business day or the next day that children are in care. And to any newly enrolled parents/guardians for 12 months from the date of this report. A signed acknowledgment of receipt of licensing report (LIC 9224) or other written statement must be placed in the child's file for verification. Page 2 A Notice of Site Visit was given and must remain posted for 30 consecutive days. Exit interview conducted and a copy of appeals rights were given to the director Lalita Gupta.
