What the inspector wrote

On 11/25/2025 at 8:41am Licensing Program Analyst (LPA) Morgan Pringle met with director Stella Wo Cabacunga for a Required - 3 year inspection . Present during the inspection were four (4) preschool age children and one (1) additional staff member. Two (2) classrooms, Tigers & Pre-K, were toured for a health and safety inspection. The facility is currently only using the Pre-K classroom. The facility operates 8:00am – 6:00pm, Monday – Friday. The facility license also has a toddler option. LPA observed the classrooms to have ample age-appropriate materials for the children’s learning and play that were observed to be clean, properly stored and in proper working order. All furniture was observed to be free from defects and in good repair. All toxins, medications, and hazardous materials were observed to be in inaccessible areas. LPA observed the bathroom to be free from defects, clean, in proper working order. There is one (1) properly working smoke/carbon monoxide detector and one (1) fully charged fire extinguisher on site. LPA observed the first aid kit as well. All items in the kit were observed to be present except for a working thermometer. LPA informed Director that a new one would need to be purchased. The facility does not transport children as well. When LPA arrived at the facility, LPA asked if the centralized heater had been turned on as it was very cold inside. LPA noted the temperature outside at the time was 48 degrees and multiple windows in building were open. LPA was informed by staff that the small space heater was being used. LPA checked the heat control panel and observed the centralized heater had not been turned. Director stated that the heater had not been used “in a while,” but turned the heater on. Page 1 of 5 LPA noted that the outside temperature had not passed 65 degrees in the last 5 weekdays. LPA asked the Director if there was a thermostat anywhere in the building as the heat/air control panel did not show the current temperature of the room. LPA was informed there was no thermostat in the building. LPA informed Director one would need to be purchased to ensure that the temperature in the building does not go below the required 68 degrees. (see LIC809D) LPA observed the kitchen to be well maintained, free from defects and all hazards have been made inaccessible to the children in care. All food on site was observed to be properly stored. The facility provides two (2) snacks and lunch is brought from the children’s home. The counters were observed to be clean and well maintained. All children bring their own water bottles and have access to clean drinking water in and outside of the classrooms. The outside play yard was observed to be fully fenced with ample age-appropriate materials for the children. All play materials for the children were observed to be clean and in good repair. LPA observed the climbing structures to have ample cushion for children’s safety and in good repair. There is a locked building used for storage in the bike area. There is plenty of shade for the children. LPA did not observe any bodies of water in or around the facility. At 9:06am LPA observed one of the wooden planks in the back fence was missing. LPA observed the hole to be wide enough for a child to climb through. Director stated the area behind the fence had trees cleared out in September and a branch must have fallen on the fence knocking the plank down. Direct stated they did not notice the missing plank. LPA informed Director the fence would need to be repaired. At 9:08am LPA observed two (2) wooden picnic tables that were cracked, had chipped paint, and the wood for the bench of the table had become detached. The two (2) plastic picnic tables in the area closest to the gate leading to the parking lot were cracked with rough edges and had tape around that had worn away. All four (4) tables had been warped and weathered from the outdoor elements. LPA informed Director that all four tables would need to be removed from the areas the children have access to. (see LIC809D) Page 2 of 5 The facility is operating within its licensed capacity and is in ratio. All staff working at the facility have obtained a criminal record clearance or have transferred their clearances. The fire/disaster drill log is complete with the last drill logged 11/12/2025. A physical census of the children was taken and cross referenced with the sign-in and out log. All children were properly signed in by their parents or authorized representatives. All required postings are posted in the entry way of the facility. LPA obtained the children’s files, staff files, and the facility files. All files were complete. Deficiencies cited during LPAs inspection · Temperature inside facility below 68 degrees · Four picnic tables in play areas in disrepair & wooden plank in fence missing Director was reminded that California law requires facilities to report unusual incidents and/or injuries to children in care, to the child's parents, and to the Department within 24 hours by phone. Within seven (7) days from the incident, facilities must submit the Unusual Incident/Injury form (LIC 624) to the Department. Director was reminded that EMSA approved Pediatric CPR & First Aid training must be completed every two (2) years. Mandated Reporter Training ("Child Care Providers") is required for all staff and is to be renewed every 2 years by visiting https://mandatedreporterca.com/ . Personnel and facility roster must be properly maintained, and fire/disaster drills must be conducted at least every six (6) months and documented. LPA informed Director that all forms can be downloaded at www.ccld.ca.gov . Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test as specified in Health and Safety Code section 1597.16. Page 3 of 5 For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP). LPA verified that the lead testing was completed in accordance to the Written Directives (Interim Licensing Standards) outlined in PIN 21-21.1-CCP . LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. For IMS information see PIN 22-02-CCP . Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for and removing any recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Page 4 of 5 Director was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.
