On November 06, 2025 at 11:10AM, Licensing Program Analyst (LPA) Lorraine Dacanay Breaux met with Director Silvia Akter for a UNANNOUNCED CASE MANAGEMENT - DEFICIENCIES . Present during LPA's visit was two (2) staff includes administrative and three (3) infants in care. The facility was toured for a health and safety inspection. Facility holds a license for preschool #073408260. During the annual visit on 09/25/2025, LPA toured the facility completed a census and during record review S2 did not have proof of qualifications via transcripts and/or teaching permit on file. LPA returned on 11/6/2025, to review education records and staff was not qualified to be with infants alone. During the annual inspection on 09/25/2025, S1 was alone with two (2) infants in care. LPA informed Director that S1 did not have proof of qualifications on file at the facility. LPA was informed that S1 was to bring the documentation, but have not done so and will soon. LPA informed Director that all staff documentation needs to be received before the staff members first day present at the facility. LPA informed Director that without proof of qualifications, S1 is considered an aide and are never to be left alone with the children in care.
