On 09/23/2025 at 9:03 AM, Licensing Program Analyst (LPA) Kareeca "Reeca" Sykes conducted an Unannounced Plan of Correction Inspection at Crystal Gatti's Family Childcare Home. LPA met with Licensee, Crystal Gatti and explained the purpose of this visit. During today's inspection, there were three (3) preschool children in care. Licensee stated there are five (5) children enrolled (3 preschool children and 2 school age). All staff caring and supervising children have Criminal Record Clearance. LPA is following up on three (3) Type A citations given to the facility on 09/11/2025 for Personal Rights. LPA reviewed files and observed in multiple children files the Acknowledgement of receipt of Licensing Report. (LIC 9224) Licensee was reminded to provide a copy of this licensing report dated 09/11/2025 that documents a Type A citation(s) to parents/guardians of all children enrolled the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of the last report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. During today's inspection, there were no violations observed. Exit interview conducted and report was reviewed with the Licensee, Crystal Gatti. A Notice of Site Visit was given and must remain posted for 30 consecutive days.
