What the inspector wrote

Licensing Program Analysts (LPAs) Jyoti Saini and Kassandra Medrano conducted an unannounced visit to the facility to deliver the final findings of the complaint investigation. During the course of the investigation, the LPAs reviewed facility records and obtained relevant documents pertaining to an incident that occurred in March 2025 . This incident was not reported to the Department nor to the families involved, as required. This practice does not comply with regulation outlined in Title 22 reporting requirements 101212 (d)(1)(C) and 101212 (f) As a result, Type B citation has been issued to the facility. Appeal rights were given. A notice of site visit was posted and must remain posted for a period of 30 days. An exit interview was conducted with Licensee, Karim Ramzanali .

    101212 Reporting Requirements (d) Upon the occurrence, during the operation of the child care center of any of the events specified in (d)(1) below, a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours. In addition, a written report containing the information specified in (d)(2) below shall be submitted to the Department within seven days following the occurrence of such event.(1) Events reported shall include the following:(C) Any unusual incident or child absence that threatens the physical or emotional health or safety of any child. (f)The items specified in (d)(1)(A) through (H) above shall also be reported to the child's authorized representative.This requirement is not met as evidenced by: Based on observations, interviews, and record reviews, the Licensee did not comply with the section cited above, as an incident that occurred at the facility in March 2025 was not reported to the Department nor to the families involved, as required by the rehualtions. This poses a potential risk to the health, safety, and personal rights of the children in care.
