This requirement is not met as evidenced by, based on the LPA's observation, during this visit, at 9:55 am, on Room 4, LPA observed S1 with 10 preschool children. LPA reviewed S1's record, and S1 only qualifies as a Teacher Aide. At 10: 01 am, S5 came in the room to supervise the children, however, S5 is also a Teacher Aide. This poses a potential risk to the health, safety or personal rights to children in care.

This requirement is not met as evidenced by, based on the LPA's observation, during this visit, at 9:59 am, LPA conducted a health and safety inspection with the Licensee Karim Ramzanali, on Room 4 preshcool bathroom, the 2 toilets appeared to be not being flushed, the water sitting in the toilet bowl appeared to still have urine in it. This poses a potential risk to the health, safety or personal rights to children in care.

101216.1 (g) Teacher Qualifications and Duties - A photocopy of the teacher's Child Development Permit as specified in (c)(3) above, or a photocopy of the teacher's transcript(s) documenting successful completion of required course work, shall be maintained at the center..... This requirement is not met as evidenced by, based on the LPA's record review at at 9am, S1 and S7 teacher’s transcripts documenting successful completion of required course work, was not maintained at the center. At 12pm, S1 and S7 had to retrieve the documentation, outside of the facility records, and provided copies to LPA for review at 2 pm. This poses a potential risk to the health, safety or personal rights to children in care.
