What the inspector wrote

Based on interviews and additional information obtained, LPA determined the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. California Code of Regulations, Title 22 is being cited on the attached LIC 9099D. Failure to submit Proof of Corrections (POC) by Plan of Correction date may result in civil penalties. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with director Regina Roces.

This requirement was met as evidence by: based on interviews it was found that a previous child in care was sent home in soiled clothing along with other incident of chidren being sent home with soiled diapers. This poses a potential risk to the health and safety of children in care.
