What the inspector wrote

Centralized Application Bureau (CAB) Licensing Program Analyst (LPA) M. Domantay met with Applicant Tamara Shek for the purpose of a pre-licensing inspection for a change of ownership. Applicant requests a single licensed child care center for a infant program ages 0 months to 24 months (capacity 7), toddler program 18-36 months (capacity 12) and preschool program ages two years to first grade entry (capacity 51) license to serve a total of 70 children. The program will operate Monday through Friday from 7:00 a.m. to 5:00 p.m. The facility is pending an updated fire clearance to approve for 7 infants 0-24 months, 12 toddlers 18-36 months, and 51 preschoolers 2 years to first grade entry from the San Ramon Valley Fire Protection Department. LPA received confirmation email from Inspector Donna Marhenke stating no changes to occupancy code E, no construction changes, change of ownership only. The facility will provide 2 snacks in the morning and afternoon and lunch will be provided by families from home. LPA reviewed with applicant the LIC 311A, Records To Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted. INDOOR ACTIVITY SPACE: There is 1 indoor area for the infant program: Classroom D, 1 toddler area: Classroom 5, and 4 areas for the preschool program: Classroom B, Classroom C, Classroom 1, and Classroom 3. LPA observed a sufficient amount of equipment, toys, tables, chairs, cubbies, cots and napping mats. Applicant plans to care for 4 crib aged infants. LPA observed 4 cribs in the crib area. LPA observed changing tables is in arms reach of a sink. There is a first aid kit on the wall by the front entrance of the facility. Medications will be stored in each classroom in a medicine box in the cabinet. LPA observed cleaning disinfectants are appropriately stored and inaccessible to children. Applicant stated there are no poisons or firearms on the premises. LPA observed a combination carbon monoxide detector on the ceiling of the front entrance. LPA observed a manual sign-in/sign-out system. Page 1 All indoor measurements remain the same from previous inspections/ownership. No constructional changes have occurred. See 812 for previous measurements. Individual measurements are recorded on the Capacity Worksheet (LIC 9024). Based on previous indoor measurements Applicant's request for 7 infant children, 12 toddler children, and 51 preschool children will be accommodated. There are 5 toilets and 5 sinks for the children, and a separate private restroom for the staff is located in the hall. Children who become ill during the day will be isolated in the office and will use the staff restroom, if necessary. OUTDOOR ACTIVITY SPACE: All outdoor measurements remain the same from previous inspections/ownership. No constructional changes have occurred. See 812 for previous measurements. Individual measurements are recorded on the Capacity Worksheet (LIC 9024). Based on previous outdoor measurements Applicant's request for 7 infant children, 12 toddler children, and 51 preschool children will be accommodated. The outdoor play area is fenced with a metal wire that is at least four feet tall. LPA observed a sufficient amount of equipment and toys. There is a wooden play structure present in the outdoor play yard area and a sandbox that was observed to be free of debris and maintained. There are no bodies of water on the premises. There is sufficient shading provided by canopies and trees for the infant, toddler, and preschool play areas. LPA notified Applicant will need to submit a request for a waiver for the outdoor play area to accommodate the maximum capacity of 7 infant children outdoors at any given time with a set schedule in place and a request for a waiver for the Toddler/Preschool outdoor play area to be shared between the two programs with a set schedule in place. Page 2 LPA discussed the following: 100% supervision is required at all times, including in the bathroom; personal rights; inspection authority; reporting requirements; staff to children ratios and capacity; staff qualifications; and maintaining buildings and grounds. LPA discussed with Applicant any changes that may occur regarding the director or an employee acting in the director's absence must be reported to department within 10 working days. Applicant was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated. Applicant was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. This facility plans to provide Incidental Medical Services – IMS. For IMS information, see PIN 22-02-CCP. A Plan of Operation that includes IMS must be submitted to the Department.
