(1) No child(ren) shall be left without the supervision of a teacher at any time...Supervision shall include visual observation. This requirement was not met as evidenced by: through interview it was found that C1 placed peas in their nose due to a lack of supervision which posed a potential risk to the health, safety and personal rights of the children in care.
