What the inspector wrote

LPA determined the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED . California Code of Regulations, Title 22 is being cited on the attached LIC 9099D. Failure to submit Proof of Corrections (POC) by Plan of Correction date may result in civil penalties. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with interim Director Vandana Anand.

This requirement was not met as evidenced by: through record review it was found that 5 staff members do not have a valid mandated reporter training. This poses a potential risk to the health, safety & personal rights of the children in care.
