What the inspector wrote

LPA determined the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED . California Code of Regulations, Title 22 is being cited on the attached LIC 9099D. A Type B deficiency is being cited today for the restrooms not being in good repair last week. Due to the facility fixing the issue the deficiency is being cleared today. There is no plan of correction needed. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with director Vandana Anand.

This requirement was not met as evidenced by: both staff bathrooms were not in good repair, which posed a potential risk to the health and safety of the employees present.
