Type B. Lower-severity citation. Corrective action required, no imminent risk. The regulator monitors compliance on the next visit.

PERSONNEL REQUIREMENTS
At least one person trained in CPR and Pediatric First Aid shall bepresent when children are at the facility or at offsite activities. This requirement was not met as evidenced by this inspection. Staff's CPR/1st Aid expired on 05/20/21. This posses a potential risk to the health, safety and/or personal rights of children in care.

A health screening as specified in Section 101216(g). This requirement was not met as evidenced by inspection. Health screening for staff #1 was missing from file. Disinfectants, cleaning solutions, poisons and other items that could pose a danger if readily available to children shall be stored where inaccessible to children. (1) Storage areas for poisons shall be locked. This requirement was not met as evidenced by LPAs inspection. Clorox Bleach, Lysol, Ajax and other cleaning supplies stored in a cabinet underneath the handwashing sink in Classroom #3-4. This posses a potential risk to the health, safety and/or personal rights of children in care.

Employees or volunteers at day care center; immunization requirements; records; exemptions. Commencing September 1, 2016, a person shall not be employed or volunteer at a day care center if he or she has not been immunized against influenza, pertussis, and measles. Each employee and volunteer shall receive an influenza vaccination... This requirement was not met as evidenced by a file review of staff files. Staff #1 did not have proof of immunization requirements. This posses a potential risk to the health, safety and/or personal rights of children in care.

This requirement was not met as evidenced by: file review of staff files. A copy of an approved Mandated Reporter Training Certificate for staff #1 was not in file. This posses a potential risk to the health, safety and/or personal rights of children in care.
