Supervision shall include visual observation. This requirement was not met as evidenced by staff interviews that indicated that child #1 was left in the outdoor play area unsupervised for approximately 5 to 10 minutes. This poses an immediate risk to the health, safety or personal rights of children in care. A civil penalty of $500 was assessed.
