What the inspector wrote

On 12/29/2023, Licensing Program Analyst (LPA) Stephanie Vega-Gonzalez conducted an unannounced Case Management inspection. The purpose of the inspection was due to an unusual indent report (UIR) that was called in to the Department on 12/05/2023. LPA met with Director, Anna Gonzales and explained the reason for the inspection. A census was taken, and LPA conducted a tour of facility inside and outside. The UIR was regarding a water flooding that had occurred in the Preschool Classroom 2 on 12/5/2023. Administrator Cordelia Dixion-Hacket had confirmed that toilets were not damaged, yet the girl\342\200\231s restroom did sustain some flooding via a phone conversation on 12/14/2023. Administrator confirmed that girl\342\200\231s restroom was repaired on 12/05/2023 and that there was no closure of restroom. Administrator confirmed that Classroom 2 was also cleaned up on 12/05/2023 and that there was no closure of classroom. On 12/29/2023 LPA inspected the boys and girl\342\200\231s restroom and observed that the boy\342\200\231s restroom was in working order. LPA observed that the girl\342\200\231s restroom had one toilet out of three that leaked when flushed. LPA informed Director, Anna Gonzales of the leakage. LPA observed that the wall base boards were missing in the boy\342\200\231s restroom, girl\342\200\231s restroom, Classroom 1, and Classroom 2. Director stated that base boards will be replaced on 01/15/2024. LPA also measured Classroom for square footage. On today's date LPA observed that the preschool playground did not have enough sand to absorb high impact areas. LPA walked on sand area throughout play ground and observed that there was no to little sand in high impact areas and felt the sand to be hard when stepped on. LPA spoke to Director, who stated that facility had applied for a grant to get funds to repair play ground area and get more sand. Director did not have an exact date of when funds will arrive. Director stated that facility will purchase more sand to add to high impact areas and will rake sand before children are allowed to play in area. LPA observed as staff began to rake sand and filled in high impact areas around the bottom of the slides during the inspection. Per California Code of Regulations, Title 22, Division 12, Chapter 1, deficiencies cited today is on the following LIC809D. During the exit interview LPA viewed the Notice of Site Visit Form that was left today being posted and left instructions that it is to be posted for 30 days.
