What the inspector wrote

On 07/09/24, Licensing Program Analyst (LPA) Valentin Hernandez, conducted an unannounced Annual Required Inspection for the preschool license. LPA met with Licensee/Director Roxan Tutelian. LPA toured the facility indoors and outdoors. Hours of operation are Monday through Friday, 7:30 am to 5:00 pm. LPA confirmed that the classrooms used for services are classrooms #1 through #6, all of which are licensed. Other licensed areas are the library, common area, and studio. Facility provides morning, afternoon and full day sessions. This facility operates year-round. LPA confirmed the phone number to the facility as 559-431-5437. Parents provide lunch for children in care. If a parent forgets to provide lunch, the parent is called, who will bring the child's lunch. Licensee provides AM and PM snacks. There is no swimming pool or other bodies of water on the premises. There are no firearms or ammunition allowed or stored on the premises. Disinfectants, cleaning solutions, medication and other hazardous items are made inaccessible. No poisons were observed during the inspection. Furniture and equipment are in good condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All toilets and handwashing facilities are in safe operating condition. Floors in the facility are clean and safe. Drinking water is available both indoors and outdoors. Licensee provides water pitchers in all classrooms. Parents provide their children with sippy cups, which are taken home daily to be washed. Areas around high climbing equipment and slides have cushioning material to absorb falls. Licensee utilizes sand for cushioning. LPA verified that fire drills are being completed at least once every 6 months. The last fire drill was conducted on 6/27/24. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. LPA verified that facility did not conduct required lead testing. Licensee is to be in accordance to the Written Directives outlined in PIN 21-21.1-CCP. (Continued on LIC 809-C) Licensee was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Capacity on license are being maintained. Bluff View Private Preschool of Fresno director or fully qualified teacher(s) designated to act in the director’s absence has been reported to the Department.This Center utilizes electronic sign in/out. All children are under supervision, including visual supervision, of a teacher at all times. During today's inspection, LPA observed staff #1, #2 and #3 with 25 preschool children. The staff were not teacher qualified. LPA informed licensee who moved staff around to be within compliance. LPA reviewed a sample of children’s files and observed that the files were complete with contact information for authorized representative and or relatives or others who can assume responsibility for the child and medical assessment. LPA reviewed a sample of staff files and observed files were mostly complete with health screening, immunization records for influenza, pertussis and measles and current documentation of completed mandated reporter training. Licensee agreed to obtain any missing documentation for her employees. This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. For IMS information see PIN 22-02-CCP. The Community Care Licensing website www.ccld.ca.gov was discussed, which will provide access to Provider Information Notices (PINs), Quarterly Updates, COVID-19 Information and Resources, Mandated Reporter Training, Safe Sleep in Child Care, Lead Poisoning Facts, Forms and Regulations. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. LPA informed licensee, Roxan Tutelian, that this report, dated 7/9/24, documents one Type A citation which shall be posted for 30 consecutive days as there is immediate risks to the personal rights, health and safety to children in care. Also, LPA Valentin informed licensee to provide a copy of today's licensing report that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, the following deficiencies were cited during today’s inspection (see LIC 809-D) . Exit interview conducted and report was reviewed with licensee Roxan Tutelian. A notice of site visit was given and must remain posted for 30 days. Appeal rights were also given.
