(b) Prior to employment, a teacher shall meet the requirements of (b)(1) or (b)(2) below:This requirement is not met as evidenced by:Based on LPA record review, the licensee did not comply with the section cited above. On 04/25/2025, Director Roxan submitted copies of transcripts and work experience documentation of the 16 staff present on 04/11/2025 and it was determined that 3 staff met requirements as qualified teachers along with 3 as qualified aides. This poses an immediate risk to the health, safety or personal rights of children in care.

(b) The licensee may use teacher aides in a teacher-child ratio of one teacher and one aide for every 15 children in attendance. (1) A ratio of one fully qualified teacher (as specified in Section 101216.1(c) and one aide for every 18 children in attendance in a preschool program is allowed when the aide meets the qualifications specified in Section 101216.2(d).This requirement is not met as evidenced by: Based on LPA observations, interviews and records review the licensee did not comply with the section cited above. During 4/11/25's inspection there were 16 teaching staff present and 142 Preschoolers in 6 classrooms. Due to unqualified staff not meeting regulatory requirements 6 out of 6 classrooms were out of ratio. This poses an immediate risk to the health, safety or personal rights of children in care.

PERSONNEL REQUIREMENTS

Based on record review, the licensee did not comply with the section cited above. There was 13 staff present during today's inspection, which none had current approved First Aid/CRP certificate. This poses a potential health, safety or personal rights risk to persons in care.

CHILD'S MEDICAL ASSESSMENTS

Based on record review, the licensee did not comply with the section cited above. Children #3, 4 & 6 are missing their TB risk assessment or TB test results from pediatrician. This poses a potential health, safety or personal rights risk to children in care.

Based on interview, the licensee did not comply with the section cited above. This poses a potential health, safety or personal rights risk to children in care.

Based on record review, licensee did not comply with the section cited above. Staff #2, 7,10, 11 and 12 are missing their TB assessments or test results. This poses a potential health, safety or personal rights risk to children in care.

Based on observation, the licensee did not comply with the section cited above. All five classrooms were out of ratio: Classroom 2: 1 Teacher & 2 Aides with 25 children, classroom 3: 3 Aides with 29 children; classroom 4: 3 Aides with 25 children; classroom 5: 1 Teacher & 1Aide with 20 children and classroom 6: 2 Aides with 20 children. This poses an immediate health, safety or personal rights risk to children in care.

The licensee shall ensure that each child is accorded the following personal rights: To be accorded safe, healthful and comfortable accommodations, furnishings and equipment to meet his/her needs. To be free from corporal or unusual punishment, infliction of pain, humiliation, intimidation, ridicule, coercion, threat, mental abuse or other actions of a punitive nature including but not limited to: interference with functions of daily living including eating, sleeping or toileting; or withholding of shelter, clothing, medication or aids to physical functioning. This requirement was not met as evidenced by the investigation findings. This poses a potential risk to the health, safety or personal rights of children in care.

This was not met as evidenced by observation that masks were not being worn at the facility which poses a potential health, safety or personal rights risk to persons in care.

CRIMINAL RECORD CLEARANCE

Criminal Record Clearance. Prior to working or volunteering in a licensed child care facility, all individuals subject to a criminal record review shall obtain a clearance or criminal record exemption. This was not met by evidence of staff record review annual inspection teacher Annelise Lyman was not finger printed. cleared. Therefore, a $500.00 civil penalty is assessed ($100.00 per violation per day for a maximum of five days by the Department).Unfinger printed teacher left the center during today's visit and another fingerprinted adult came to the class to assist. Licensee was given the Live Scan form during today's visit. Licensee to send a copy of the completed Live Scan form/receipt by 2/13/18 showing that assistant was fingerprinted. Licensee understands that her assistant cannot be present in the home until fingerprint cleared or have a clear exemption and is associated with her facility.This poses an immediate threat to the health and safety or personal rights of children.A civil penalty was assessed for $500 due to teacher has been working at the school for longer than 30 days.

Personnel Recordsa) The licensee shall ensure that personnel records are maintained on the licensee, administrator and each employee.
