Inspector’s narrative

What the inspector wrote

On 03/15/2024 Licensing Program Analysts (LPAs) Miguel Herrera, and Elizabeth Torres conducted an unannounced Case Management inspection. Today, LPAs met with Director Beth Nelson and toured the facility, and a census was taken. The reason of this inspection is to discuss an Unusual Incident that occurred on 02/12/2024. According to staff child #1 got a small ball (crafting ball) from the sensory table lodged in his nose. Staff immediately removed the balls from the sensory table. Per Director Nelson, children are under supervision while using the sensory table and are only allowed to use the sensory table with direct supervision from a staff member. Director Nelson stated that items in the sensory table are age appropriate to each class. During the facility tour LPAs observed the sensory table to be closed with a lid. Upon inspecting the sensory table LPAs did not see any items that posed a risk to children in care. This appears to be an isolated incident and staff took appropriate measures to address the incident. Per Director Beth Nelson, she will incorporate items that do not pose a risk to a child into the sensory table. Director Nelson stated that she reminded staff to supervise children while using the sensory table. Per the California Code of Regulations, Title 22, Division 12, Chapter 1, no deficiency has been cited during today’s inspection. Director Beth Nelson was provided a copy of appeal rights. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with Director Beth Nelson.
