101223 Personal Rights (a)The licensee shall ensure that each child is accorded the following personal rights:(1) To be accorded dignity in his/her personal relationships with staff and other persons.This requirement was not met as evidenced by: Based on interviews and documents reviewed as noted on LIC809, Staff #1 innapropiately handled child #1 by lifting the child by the wrist while the child was resisting and upset. This posed an immediate risk to the health, safety, or personal rights to children in care.

(a) The licensee shall ensure that each child is accorded the following personal rights:(2) To be accorded safe, healthful and comfortable accommodations, furnishings and equipment to meet his/her needs.This requirement was not met evidenced by: Based on interviews and document review, Child #1 sustained an injury while in care that required medical attention. Per interviews, child #1 was hanging from the soccer goal nets and the goal posts flipped and landed on the child. Per interviews, the goal posts were not secured and anchored to the ground during the incident, which posed an immediate risk to the health, safety, and personal rights of children in care.

Supervision shall include visual observation. This requirement is not met as evidenced by:Based on interviews, the licensee did not comply with the section cited above. During today's inspection LPA gathered information that staff left a child unattented for approximately 2 minutes which poses a potential health, safety or personal rights risk to persons in care.
