Facility-requested visit·January 14, 2025

On January 14, 2025 at 12:35 PM Licensing Program Analyst's (LPA's) Gloria Gonzalez and Jacqueline Macias conducted an unannounced, Licensee initiated, case management inspection to add an enclosed patio which includes a room and a bathroom for daycare use and met with Licensee, Elsa Lizarraga.\302\240 LPA disclosed the purpose of the inspection and was granted entry by the Licensee. The Licensee, Licensee's mother, Licensee's daughter and Licensee's grandchild, were present at the facility during this inspection. On 2/28/24, Licensee submitted documentation requesting to enclose her patio for daycare use. The enclosed patio includes a room and a bathroom and has a door that leads to the backyard. On 11/15/24 Licensee advised of the completed work and submitted a copy of an updated application, facility sketch, and the city permit. On 1/3/25 the department received a granted Fire Clearance signed on 12/16/24. Licensee accompanied LPA on a tour of the home and the enclosed patio (Daycare area). LPA's did not see any hazardous items and is safe for daycare children to use. This facility is now a 2 story, 5 bedroom, 4 bathroom house with a studio/garage and 1 bathroom. The following areas used for child care are: The living room, hallway bathroom (located downstairs), the kitchen, the dining area, tv room, the new enclosed patio (daycare area that includes a room and a bathroom) and the backyard. The following areas are kept inaccessible with the use of locks and safety gates: 4 bedrooms and 2 bathrooms that are located on the second story and the attached studio/garage with 1 bathroom. A copy of the report and appeal rights (LIC 9058) and notice of site visit (LIC9213) was provided to Licensee and must remain posted for 30 days and advised must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. LPA interpreted and explained the inspection report to licensee in Spanish, licensee stated she understood. An exit interview was conducted and report was reviewed with the licensee, Elsa Lizarraga.
