On August 22, 2025, an Informal Office Meeting was conducted at the Fresno South Regional Child Care Office. In attendance at the meeting was Licensing Program Manager, (LPM) Cynthia Brannon, Licensing Program Analyst, (LPA) Octavia Nolan, and Licensee, Amanda Guinn. The purpose of this meeting was to discuss recent violations of Title 22 regulations, that if not corrected, would pose an immediate and potential risk to the health, safety, and personal rights of children in care. The following issues/violations was discussed: Type B Deficiency cited on 08/05/2025 101229(a)(1) Responsibility for Providing Care and Supervision Based on observation and interviews conducted during a Case Management inspection, the Licensee left children alone in the preschool classroom without the supervision of a teacher. Type B Deficiency cited on 08/05/2025 101161(a) Limitations on Capacity Based on observation and interviews conducted during a Complaint Inspection, the Licensee provided care and supervision to two school age children at the facility during operating hours. Continued on LIC 809C Type B Deficiency cited on 05/12/2025 101161(a) Limitations on Capacity Based on observation, interviews conducted, and records obtained, staff provided care to infants and school age children at the facility during operating hours. Type B Deficiency cited on 04/28/2025 101229.1(b) Sign in and Sign Out Based on observation and records review of the sign in/ out sheet, two children were removed from the facility without being signed out. Type B Deficiency cited on 04/28/2025 101239(o)(1) Fixtures, Furniture, Equipment and Supplies Based on observation during a Case Management Inspection, children had access to a playhouse with chipping paint and rust on the handrails. The stairs to the playhouse were not sturdy. An 8-inch gap was observed in the fencing that leads to an unlicensed area. Wooden poles in the outdoor area were splintered and chipped. Type B Deficiency cited on 04/28/2025 101239(e)(4) Fixtures, Furniture, Equipment and Supplies Based on observation during a Case Management Inspection, the children\342\200\231s restroom was used to store supplies such as decorations, toys, and fire extinguishers. Type A Deficiency cited on 03/20/2025 101216.3(a) Teacher-Child Ratio Based on observation during an Annual Random Inspection, one facility staff was supervising 13 children for approximately 30 minutes during the inspection. Continued on LIC 809C Type B Deficiency cited on 03/20/2025 101161(a) Limitations on Capacity Based on observation and interviews conducted during an Annual Random Inspection, staff provided care and supervision to an eight-month-old infant during operating hours. Corrections for all deficiencies were received at the Fresno Regional Office before August 22, 2025. The facility has been referred to the Technical Support Program (TSP) for assistance. It was discussed that continued violations of Title 22 Regulations and failure to maintain compliance will result in a Non-Compliance conference and may be referred to our Legal Division for possible Administrative Action.
