What the inspector wrote

On 01/15/2026, Licensing Program Analyst (LPA), Christopher Burnias met with Owner, Laurae Raths for an unannounced Case Management incident inspection. LPA toured the facility, and a census was taken. The purpose of today's inspection was to address two unusual incidents that were reported to the Fresno Community Care Licensing (CCL) office. LPA interviewed Staff 1 (S1), who was present during an Unusual Incident that occurred at the facility. Information obtained from interviews indicated that on 10/16/25 at or around 9:40 AM Child 1 (C1) threw a chair inside Classroom 2 . According to S1, C1 became upset with them after being told they could not take a toy from another child. C1 began yelling, hit S1, and then threw a chair. S1 stated that the chair did not strike anyone and that other children were moved to another part of the classroom. S1 stated that they made attempts to redirect C1’s behavior by offering other play activities but C1 continued to yell and be upset. S1 stated that after C1 threw the chair, they calmed down, gave them a hug, and went on to continue playing. S1 stated that they have talked to C1’s parent about the behavior and have come up with a plan to address challenging behaviors. S1 stated that the plan has been effective in redirecting C1 when they begin showing angry behaviors. S1 stated that at the time of incident, they were supervising 12 children. **Continued on next page** LPA also interviewed S1, and Staff 2 (S2) regarding a second Unusual Incident involving C1 at the facility. Information obtained from interviews indicated that on 10/17/25 at or around 9:40 AM C1 was spinning inside the classroom, fell, and hit their lip on a table. According to staff, C1 was observed spinning around in Classroom 2. Both S1 and S2 had instructed C1 several times to stop spinning and attempted to redirect C1’s behavior. C1 continued spinning, lost their balance and fell hitting their lip against a table. S1 assessed the child for any injuries and noticed a small cut on their lip with minor bleeding and swelling. S1 cleaned C1’s wound and applied an ice pack to the injury. Medical attention was not necessary. Owner notified parent of the incident. S1 stated that since the incident, a plan has been established with C1’s parent on effective redirection for C1. S1 stated that the plan has been mostly effective, however during the times where C1 does not want to stop spinning, S1 will move around to ensure that C1 will not bump into anyone or anything. Owner stated that at the time of the incident S1 and S2 were supervising 12 children. LPA inspected classroom area where C1 fell. LPA did not observe any damaged or broken furniture or observe any tripping hazards present. After interviewing staff members about the two unusual incidents, LPA has determined that the facility was within ratio requirements. LPA also observed locations where staff and children were positioned for the unusual incidents and LPA has determined that adequate supervision was provided despite the individual incidents occurring. Staff took appropriate measures to assess each incident, and no regulations were violated. Per Title 22, Division 12, Chapter 1 of the California Code of Regulations, no deficiencies are cited during today's visit. An exit interview was conducted with Owner, Laurae Raths and appeal rights were provided. This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit is provided and is required to be posted for 30 days.
