What Happened
Case management visit identified critical supervision violations. Children using restrooms without teacher supervision, especially on Mondays/Fridays with reduced staffing. Facility failed to submit required Unusual Incident Report for 10/16/2024 incident within 7-day deadline.
Observations
- Children observed in separate restrooms without visual teacher supervision
- Teacher observed walking between bathroom doorways without maintaining continuous visual observation
- Children confirmed no teacher presence when using restroom, particularly on Mondays/Fridays with fewer staff
- Unusual Incident Report (UIR) for 10/16/2024 incident involving inappropriate actions between two children and staff member not submitted within required 7 days
