On 09/02/2025, an unannounced case management visit was conducted at the facility by Licensing Program Analyst (LPA), Sebastian Phouthavong, who met with Site Supervisor, Michelle Walker. Previously on 07/15/2025 during an annual/Random inspection, an error occurred where the LIC809-D Page did not provide any detailed information on the report. LPA amended the LIC809-D Page and requested the document be signed by a facility representative. During the inspection the facility was toured inside and outside. The Site Supervisor and one staff were supervising eight children and operated within the licensed capacity and ratio requirements. LPA and the facility received physical copies of the amended LIC809-D Page with Site Supervisor signature. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Site Supervisor, Michelle Walker.
